Job summary

Centene Corporation is seeking a Care Management Professional in Texas to assess and coordinate services for complex, high-acuity members. The role includes developing individualized care plans and educating families to access appropriate benefits.

The hybrid position may require in-person assessments within Bell and Brazos counties and ongoing collaboration with care teams to ensure person-centered care and regulatory compliance. RN or NP licensure is required; RUG certification is a plus.

Qualifications

• Requires RN or NP licensure with state licensure.
• 4–6 years of related experience in care management.
• Bachelor's degree in Nursing preferred.
• Ability to travel within service area.

Responsibilities

• Evaluate needs and recommend a care plan for high-risk members.
• Coordinate with care teams to arrange providers and services.
• Monitor and update care plans for changes in condition.
• Educate members and families about services and benefits.
• Perform home or site visits as needed.
• Ensure compliance with state and federal guidelines.
