Centene seeks a licensed nurse to perform care management duties for complex/high acuity populations in Texas, delivering in‑home assessments and developing personalized care/service plans to promote quality, cost‑effective care. Requires RN or NP licensure in Texas, a Bachelor’s or nursing degree with 4–6 years of related experience; RUG certification within 90 days.

This is a hybrid role with in‑home visits and collaboration with care teams.

Qualifications

• Graduate from an Accredited School of Nursing or possess a Bachelor's degree with 4–6 years related experience.
• RN license required or NP license required.
• RUG certification must be obtained within 90 days of hire

Responsabilités

• Assess and coordinate care for the most complex/high risk/high acuity members.
• Develop and continuously assess long‑term care plans/service plans with care management team.
• Coordinate between member/family and care provider teams to ensure person‑centered care.
• Monitor progress and revise care plans to meet member goals/needs.
• Educate members and families on disease processes, services, and benefits.
• Act as liaison between members, families, physicians, and facilities/agencies.
• Identify and connect with community resources and services as needed.
• Document and maintain all member information and care activities per regulations.
• Perform in‑home assessments as required and collaborate with leadership to improve quality of care.
• Support onboarding and coaching of new clinical hires as needed.
