Job summary

Centene Corporation in Texas is seeking a licensed RN or NP Care Manager to assess and coordinate care for complex, high-acuity members. This hybrid role includes in-home assessments and developing personalized care plans, collaborating with families and providers to ensure high-quality, cost-effective services.

You will monitor plans, educate patients, and navigate community resources, while complying with state and federal guidelines. Requires Texas license and 4–6 years of relevant experience.

Qualifications

• Graduate from an Accredited School of Nursing or a Bachelor's degree and 4–6 years of related experience.
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
• NP - Nurse Practitioner - Current State's Nurse Licensure required
• RUG certification must be obtained within 90 days of hire

Responsibilities

• Evaluate complex/high-risk members and develop care plans.
• Coordinate with care teams to address member needs and services.
• Monitor and revise care plans based on status and goals.
• Educate members and families about services and benefits.
• Act as liaison between members, families, physicians, and facilities.
