Job summary

Centene is seeking a hybrid RN Care Manager to coordinate care for complex/high-acuity members in the Hidalgo County area, including home visits and LTSS coordination. You will develop personalized care plans, educate families, and collaborate with care teams to ensure high-quality, cost-effective services.

The role supports a Monday–Friday schedule, with mileage reimbursement for visits, and requires RN licensure with 4–6 years of clinical nursing experience.

Qualifications

• Requires Graduate from an Accredited School of Nursing and 4–6 years of related clinical nursing experience.
• RN licensure and/or NP licensure current in state.
• RUG certification must be obtained within 90 days of hire.

Responsibilities

• Evaluates the service needs of the most complex or high risk/high acuity members and recommends a plan for the best outcome.
• Develops and continuously assesses long-term care/service plans with care management teams.
• Coordinates between members/families and care providers to ensure person-centered care.
• Monitors and revises care plans to meet goals and changes in condition.
• Reviews referrals and intake data to inform care plans.
