Qualifications

• Requires RN or NP licensure with state or compact licensure.
• RUG certification must be obtained within 90 days of hire.
• Bachelor's degree in Nursing with 4‑6 years related experience preferred.

Responsibilities

• Perform care management duties to assess and coordinate services for complex/high acuity populations.
• Develop and monitor long‑term care plans and collaborate with providers and community resources.
• Coordinate with member, families/caregivers and care team to deliver person‑centered care.
• Monitor and update care plans as member status changes and goals evolve.
• Act as liaison between member/family and providers and educate on benefits and services.
