Responsibilities

• Evaluate member needs and barriers to care, coordinating with providers to address gaps.
• Develop and update person-centered care plans and connect with community resources.
• Provide ongoing follow-up and monitor progress toward plan goals.
• Coordinate between member, family, and care team to ensure timely access to services.
• Educate members and families about disease processes, care options, and benefits.
• Perform telephonic, digital, or in-person outreach as needed.
