Job summary

Centene Corporation is seeking a Remote/Hybrid Nurse Care Coordinator to assess and coordinate care for high acuity members across the continuum. You will develop personalized care plans, educate families, and act as liaison among members, providers, and facilities.

The role requires RN licensure, 4–6 years of related experience, and preferred BSN. Schedule is Monday–Friday with mileage reimbursement and travel for member visits; remote/hybrid options available.

Qualifications

• Graduate from an Accredited School of Nursing
• 4-6 years of related RN/care coordination experience
• Bachelor's degree in nursing preferred

Responsibilities

• Evaluate the needs of high risk/acuity members and recommend care plans
• Develop and assess long-term care plans with care team and providers
• Coordinate between member/family and care team to ensure appropriate care
• Monitor progress toward care plan goals and revise as needed
• Collaborate with providers to update care plans for member needs
• Maintain member data to ensure compliance with guidelines
• Educate members and families on diseases and services
• Serve as liaison between member, physician, and facilities
• Travel for member visits and required site visits
