Job summary

APA Family Support Services seeks a Lead Care Manager to coordinate care for the ECM program. You will provide direct services, develop care plans, and advocate for clients across health and social services in San Francisco.

The role requires experience in care management, EMR usage, and strong communication. Office, telehealth, and field-based options are available; occasional travel may be needed.

Qualifications

• Demonstrated interpersonal skills to build trust with clients and families.
• Experience conducting home visits and outreach activities.
• Strong organizational abilities to manage schedules and care plans.
• Physical capability to lift/move up to 25 pounds as needed.

Responsibilities

• Assess client needs across physical/mental health, SUD, housing, and wellness.
• Develop and implement individual care/action plans with timely documentation.
• Provide services at home, office, telehealth, or field locations.
• Coordinate with healthcare professionals and community supports.
• Apply motivational interviewing, harm reduction, and trauma-informed care.
• Collaborate with hospital staff on transitional care planning.
• Engage clients through outreach, calls, mailings, and visits.
