You will partner with hospitals and community providers, oversee budgets, quality, and growth, and pilot innovations such as telehealth, while traveling 60–70% to counties to support equity and care transitions.

• Lead Team Leadership & supervision across multiple counties and mentors Lead Care Managers.
• Oversee program development, quality assurance protocols, and standardized workflows.
• Ensure high-quality, in-person member visits and robust documentation support.
• Serve as liaison between ECM program and hospitals, providers, and community organizations.
• Develop and manage program budgets, allocate resources, and track expenditures.
• Identify opportunities to enhance program effectiveness and pilot innovations (e.g., telehealth).
• Lead operational huddles, quarterly reports, and stakeholder communications.
