Job summary

Front St. family of companies is seeking a licensed or registered Lead Care Manager for its Enhanced Care Management (ECM) program in Santa Cruz, CA.

You will deliver in-person care management and care coordination to Medi-Cal members with complex medical, behavioral health, and social needs, ensuring compliance with CCAH and DHCS requirements. Responsibilities include managing an assigned ECM caseload, conducting comprehensive assessments, developing care plans, and providing transitional care

Qualifications

• Must hold one of the active California clinical credentials listed under Eligible Clinical Credentials.
• One year of experience serving adults with serious mental illness, substance use disorders, chronic health conditions, or homelessness.
• Bilingual English/Spanish preferred.

Responsibilities

• Lead Care Manager for an assigned ECM caseload coordinating physical health, behavioral health, substance use, dental, and social services.
• Conduct and document outreach to engage ECM members, primarily in person.
• Develop, update, and monitor each member's Care Management Plan with review by Clinical Director.
