Job summary

On Lok in San Francisco seeks a Lead Care Manager 1 to coordinate health care resources, develop care plans, and educate members within ECM/PACE programs. You will act as clinical advocate and liaison with providers, performing home visits and coordinating care for at‑risk populations.

The role requires a BS in Social Work or related field, 3 years in case management, HIPAA knowledge, strong communication, and proficiency with MS Office and EHR systems; remote and field work possible as part of

Qualifications

• Bachelor's Degree in Social Work, Behavioral Health, or Public Health required.
• 3 years experience in case management and/or social services setting.
• Knowledge and understanding of HIPAA regulations.
• Strong communication skills and ability to engage with members and families.

Responsibilities

• Coordinate health care resources and develop care plans for a panel of members.
• Perform home visits and on-site assessments; update care plans and referrals.
• Serve as a liaison between members, physicians, and community providers.
• Document all member interactions and care plans in the EHR system in a timely manner.
• Participate in team meetings and trainings to support care coordination.
