Job summary

Episcopal Community Services (ECS) seeks an ECM Lead Care Manager to enhance care coordination and health outcomes for high-need participants. The role is fully onsite, with travel to community sites and a focus on housing, behavioral health, and social services integration.

You will lead a multidisciplinary team, advocate for clients, and ensure compliant, person-centered care plans. ECS offers healthcare benefits and a mission-driven environment in San Francisco.

Qualifications

• Bachelor’s degree in behavioral health, human services, public health, or related field preferred.
• Two years’ experience in housing navigation, social services, or a related field with care management focus.
• Community Health Worker certification preferred but not required.

Responsibilities

• Conduct outreach to identify eligible clients for ECM services and engage them using multiple strategies.
• Interview and assess medical, behavioral, and social needs of clients.
• Develop and monitor personalized care plans with clients, families, providers, and resources.
• Coordinate multi-disciplinary care team meetings and ensure integrated care delivery.
• Document services promptly in the EHR and maintain concise case records.
• Travel locally to deliver services at client sites and during community outreach.
