Job summary

Exceptional Parents Unlimited, Inc. seeks a Lead Care Manager to provide family-centered ECM care coordination in California. You will coordinate with providers, oversee care plans, and advocate for members.

Responsibilities include engaging ECM-eligible members, assisting with discharge planning, linking to community resources, and documenting the care team. A California driver’s license and strong communication skills are required; bilingual abilities are a plus.

Qualifications

• High school diploma or equivalent with relevant social services coursework or related experience.
• 2+ years' experience in case management with knowledge of community resources.
• California driver's license and reliable transportation.
• Bi-lingual in English to Spanish, Hmong, Thai or Laotian a plus.

Responsibilities

• Coordinate with providers to ensure seamless services.
• Engage ECM-eligible members and oversee care plans.
• Assist with discharge planning and hospital coordination.
• Advocate for members and promote health self-management.
• Maintain confidential records per HIPAA and related regulations.
