Job summary

BLEHEALTH is seeking a Lead Care Manager to coordinate care for chronically ill and high-risk members in the South Bay area within the ECM program. This role involves conducting field visits, coordinating with providers, families, and community resources, and ensuring adherence to Medi-Cal ECM guidelines.

The ideal candidate will have strong care coordination, time management, and communication skills, with a valid CA driver’s license and CPR certification.

Qualifications

• Valid California driver's license, auto insurance, and a clean driving record.
• Reliable personal vehicle within a 20-mile radius.
• Ability to visit hospitals, member homes, and community locations for ECM services.
• Negative TB test and CPR certification.
• Live Scan background check completion.
• Ability to meet daily productivity expectations.

Responsibilities

• Coordinate care across clinics, hospitals, specialists, and community agencies to ensure seamless service and avoid duplication.
• Oversee ECM services delivery and ensure implementation of individualized care plans.
• Provide services where the member lives and seeks care with cultural awareness.
• Assess unmet medical, behavioral, and social needs and develop comprehensive care plans.
• Support access to medical, behavioral health, and specialty care, and arrange transportation.
• Accompany members to office visits, maintaining professional boundaries.
• Monitor treatment adherence and medication compliance.
• Provide health promotion and culturally/linguistically appropriate education.
• Promote timely access to care and reduce ER visits and readmissions through quality improvement.
• Use motivational interviewing and trauma-informed care to build trust.
• Increase member capacity for self-management and shared decision-making.
• Connect members to community resources to improve health and well-being.
• Apply crisis navigation skills for urgent or complex needs.
• Serve as primary point of contact for members, caregivers, providers, payers, and partners.
• Maintain relationships with primary and specialty providers for transitions of care.
• Work with hospital staff on discharge planning and follow-up.
• Facilitate and attend meetings with members and providers.
• Communicate with members via face-to-face, email, phone, text, and other approved methods.
• Work independently and with diverse teams, showing teamwork and organization.
• Identify high-risk members and add to registry or EHR.
• Monitor care plan adherence and adjust plans as needed.
• Maintain accurate documentation and meet productivity expectations (min 30 schedules per day).
• Attend ECM trainings and meetings.
• Provide feedback to support program improvement.
• Ensure services align with MCP guidelines.
• Use Microsoft Office and other systems to support documentation and reporting.
• Handle PHI in compliance with HIPAA.
• Follow BLE Health’s password and data-security protocols; limit personal tasks and errands to designated breaks.
