Job summary

Sacramento Children's Home seeks a Care Manager Lead to provide case oversight, supervision, and operational leadership for the ECM team. You will balance leadership with direct care and maintain a caseload of about 25 ECM members, ensuring CalAIM ECM fidelity and compliant documentation.

Responsibilities include supervising staff, coordinating medical and social services, and driving quality improvement, with after-hours support as needed. Strong leadership and collaboration are essential.

Qualifications

• High school diploma or GED is required; advanced degree preferred.
• Active California licensure in a related field is preferred.
• Three years’ experience in care coordination, case management, or behavioral health services preferred.

Responsibilities

• Provide individual and group supervision to Care Managers.
• Review assessments, care plans, progress notes for accuracy and compliance.
• Maintain an active ECM caseload and ensure fidelity to CalAIM ECM requirements.
• Coordinate medical, behavioral health, social services, housing, education, and community resources.
• Lead program meetings and participate in quality improvement activities.
• Document accurately and ensure timely reporting; participate in after-hours as needed.
