Exceptional Parents Unlimited in California seeks a Lead Care Manager to provide family-centered, member-focused care coordination for children enrolled in Enhanced Care Management (ECM). The LCM coordinates with providers, supports care plans, and advocates for patients within managed care guidelines.

The role involves in-home or community visits, collaborating with multidisciplinary teams, monitoring adherence, and promoting self-management.

Qualifications

• High school diploma or equivalent; 12 units in Social Services or related field
• 2+ years of case management experience
• Valid California driver’s license with clean driving record
• Bilingual English with Spanish, Hmong, Thai, or Laotian is a plus
• Proficient computer skills and Microsoft Office knowledge

Responsabilités

• Coordinate with members’ service providers to ensure seamless care and prevent duplication of services
• Engage ECM-eligible members and oversee care plan implementation
• Advocate for ECM members with healthcare professionals
• Conduct home, office, or community visits as needed within guidelines
• Monitor treatment adherence and medication usage
• Maintain confidential family records per HIPAA and other regulations
