Job summary

Fresno Mission is seeking a Lead Care Manager to provide family-centered, relationship-based care coordination for ECM-eligible individuals. You will assess patient conditions and coordinate resources, options, and care plans to promote healthy outcomes under CalAIM.

Qualifications

• Requires an AA or higher in Drug and Alcohol, Social Work, Human Services or related field.
• Ability to coordinate with service providers and support networks.
• Respect for HIPAA and confidentiality.

Responsibilities

• Coordinate with member’s service providers and support network to ensure seamless experience and avoid duplication of services.
• Actively engage ECM-eligible members and oversee ECM services and care plan implementation.
• Offer services where the member lives or seeks care, following managed care guidelines.
• Connect ECM members to social services and supports, including transportation.
• Advocate for ECM members with health care professionals and utilize motivational interviewing and trauma-informed care.
