ArchWell Health in Birmingham, AL is seeking a Care Navigator to create a positive experience for members and their families from arrival to departure. The role focuses on welcoming, guiding, and coordinating care with a multi-disciplinary team, while maintaining accurate records and strong customer service.

Ideal candidates have a passion for senior care, excellent communication, and the ability to work with diverse populations.

Competenze

• Associates degree preferred or equivalent experience.
• At least 1 year of work experience in a clinical setting, or similar.
• Experience with EMR systems is a plus.

Mansioni

• Welcome members and provide a welcoming check-in experience.
• Update member data in the EMR system.
• Schedule appointments and coordinate referrals for a multi-disciplinary care team.
• Assist with paperwork via electronic kiosks.
• Request medical records and upload to EMR.
• Answer questions from members and families.
• Manage center phone line and relay messages.
• Assist with center events as needed.
