Qualifications

• RN Diploma or Nursing degree with 2+ years in community-based services.
• Bachelor's degree in Social Work with 1-2+ years in community-based services.
• 5+ years of clinical experience.
• 1-2+ years supervisory experience of professional staff.

Responsibilities

• Oversee two teams of Administrative Support Specialists providing Care Coordination referral support and admin support to Care Coordinators.
• Provide member customer service support across Medica's programs.
• Partner with Clinical Care Manager to implement and evaluate care coordination services.
• Identify improvement opportunities and manage corrective action plans as needed.
