Qualifications

• RN Diploma or Degree in Nursing is required.
• Bachelor's degree in Social Work is required.
• Minimum 2 years clinical experience and 1–2 years supervisory experience are required.
• Active RN or SW license in MN is preferred or required.

Responsibilities

• Supervise care coordinators and mentor the team.
• Educate the team on using membership information for care interventions.
• Manage assignment of members to care coordinators.
• Monitor team performance and compliance with regulations.
• Participate in audits and improvement plans.
