Job summary

Medica is seeking a Nursing/Social Work leader to manage a team of care coordinators for State Public Programs and other Medica members. You will provide direct supervision, mentor staff, and ensure coordinated care across MS0, MSC+ and SNBC products.

The role includes oversight of clinical care, performance monitoring, and collaboration with multiple Medica teams. Required RN or SW licensure in MN with 2+ years in community-based services and supervisory experience; MnCHOICES certification

Qualifications

RN Diploma or Bachelor degree and 2+ years in community-based services; supervisory experience
Experience leading a care coordination team or equivalent
MnCHOICES certification or ability to obtain within 21 days preferred
Active RN or Social Work license in MN required

Responsibilities

Supervise the care coordinators and mentor team members
Educate the team on using membership information to prioritize and coordinate care
Manage care coordination for vulnerable populations and ensure regulatory compliance
Monitor performance, audits, and adherence to care models
Participate in on-call program and cross-functional collaboration
