Job summary

The Supervisor will lead Care Coordination and Engagement teams for State Public Programs (MSHO, MSC+, SNBC) and other members, collaborating with the Clinical Care Manager to drive quality care. Office-based in Minnetonka, MN, with on-site work about 3 days per week.

Qualifications

• Active, unrestricted RN OR SW license in MN required.
• MnCHOICES Certification preferred or obtainable within 21 days of employment.
• Public Health Certification preferred.
• 5+ years clinical experience.
• 1–2+ years supervisory experience of professional staff.

Responsibilities

• Oversee a team of Care Coordinators and Engagement Coordinators.
• Provide care coordination outreach and services to unengaged members.
• Partner with Clinical Care Manager for delivery and evaluation of care coordination.
• Ensure regulatory and quality compliance across programs.
• Lead improvement opportunities and mentor staff.
