Job summary

Medica is seeking a Supervisor, Clinical Care Support Specialist Teams in Minnetonka, MN to lead two care coordination support teams and partner with the Clinical Care Manager. The role focuses on implementing and evaluating care coordination services for State Public Programs and other members.

The position requires an RN or Social Work degree with substantial clinical and supervisory experience, and includes onsite work three days per week in our Minnetonka office.

Qualifications

• RN Diploma or Degree in Nursing plus 2+ years in community-based services or
• Bachelor's degree in Social Work plus 1–2+ years in community-based services
• 5+ years of clinical experience
• 1–2+ years supervisory experience of professional staff

Responsibilities

• Oversee two teams of Administrative Support Specialists providing Care Coordination referral and department support
• Provide member customer service support and remote team leadership
• Partner with Clinical Care Manager to implement and evaluate care coordination services and improvements
