Job summary

Qualifications

• High School diploma or GED equivalent required.
• BA/BS degree preferred or equivalent experience considered on a year-for-year basis.
• Minimum 3 years of work experience; Medicare Advantage knowledge is a plus.

Responsibilities

• Advocates for patients and connects eligible individuals with health plans and community resources.
• Proactively reaches out to disenrolled members to facilitate their return to care.
• Works with center leadership, sales teams, brokers and clinical staff to retain patients.
• Ensures seamless transitions to prevent gaps in care and hospital visits.
• Documents patient interactions in EMR and Salesforce, maintaining HIPAA compliance.
