Job summary

ChenMed is seeking a Patient Continuity and Needs Advocate (PCNA) to bridge patients with comprehensive care, ensuring eligible patients receive the right healthcare plans and support services. This role supports seniors, reduces care disruptions, and strengthens patient relationships.

You will work with center leadership, brokers and clinical staff to retain patients, document in EMR systems including Salesforce, and ensure HIPAA-compliant handling.

Qualifications

• High School diploma or GED required.
• BA/BS degree preferred.
• Minimum of three years’ work experience; Medicare Advantage knowledge is a plus.

Responsibilities

• Patient Advocacy & Connection: Evaluates patient needs and connects eligible individuals with health plans and community resources.
• Retention & Re-engagement: Proactively reaches out to disenrolled members to facilitate their return to our centers.
• Collaborative Partnership: Works with center leadership, brokers and clinical staff on retention strategies.
• Care Continuity: Ensures seamless transitions and sustained engagement to prevent gaps in care.
• Documentation: uses EMR, Medals and Salesforce to document patient issues and resolutions; maintain queues.
• Complies with HIPAA and other regulatory requirements; performs additional duties as assigned.
