Job summary

ChenMed in Prairie Township, OH is seeking a Patient Continuity and Needs Advocate (PCNA) to bridge patients with care, health plans, and community resources. This role ensures eligible individuals receive tailored support and access to benefits.

The PCNA collaborates with center leadership and brokers, enhances patient-provider relationships, and maintains accurate records in EMR, Medals, and Salesforce, while adhering to HIPAA and quality standards.

Qualifications

• High School diploma or GED required.
• BA/BS degree preferred or equivalent experience considered.
• Minimum three years’ experience; Medicare Advantage knowledge a plus.
• Experience with seniors is beneficial.

Responsibilities

• Advocate for patients and connect eligible individuals with health plans and resources.
• Proactively reach out to disenrolled members to facilitate their return to care.
• Collaborate with center leadership, sales brokers, and clinical staff on retention strategies.
• Ensure seamless care transitions and sustained engagement to prevent gaps.
• Document patient issues and resolutions in EMR, Medals and Salesforce with HIPAA compliance.
• Perform other duties as assigned by management.
