Job summary

ChenMed is expanding and seeks a Patient Continuity and Needs Advocate to bridge patients with health plans and services. The role focuses on retaining and re-engaging seniors, coordinating with centers, brokers, and clinicians for continuous care, and documenting in EMR systems.

Strong customer service, healthcare knowledge, and English communication are essential. High school diploma required; BA/BS preferred or comparable experience.

Qualifications

• High School diploma or GED required.
• BA/BS degree preferred or additional relevant experience.
• Minimum of three years' work experience; seniorcare/Medicare Advantage understanding a plus.
• Fluency in English; strong written and verbal communication.

Responsibilities

• Advocate for patients and connect them with health plans and community resources.
• Proactively reach out to disenrolled members to re-engage care.
• Collaborate with center leadership, sales, brokers and clinical staff.
• Ensure care continuity and minimize gaps that lead to hospital visits.
• Document patient interactions in EMR, Medals and Salesforce; HIPAA compliant.
