The PCNA collaborates with center leadership, sales teams, brokers and clinical staff to retain patients, reduce care disruptions and document issues in EMR, Medals, and Salesforce while maintaining HIPAA compliance.

Qualifikationen

• High School diploma or GED required; BA/BS preferred.
• Minimum 3 years work experience, especially with seniors or Medicare Advantage.
• Excellent customer service, communication and problem-solving skills.

Aufgaben

• Advocate for patients and connect them with health plans and resources.
• Proactively reach out to disenrolled members to re-engage care.
• Collaborate with center leadership, sales teams, brokers and clinical staff.
• Ensure care continuity and prevent gaps that could lead to hospital visits.
• Document patient issues and resolutions in EMR/Medals/Salesforce; HIPAA compliant.
• Perform other duties as assigned by management.
