Job summary

ChenMed LLC is hiring a Patient Continuity and Needs Advocate (PCNA) to act as the bridge between patients and comprehensive care. The role focuses on connecting eligible patients with plans and services while maintaining strong relationships and improving care outcomes.

The PCNA will collaborate with center leadership, brokers, and clinical staff to reduce care disruption, support disenrolled patients, and document care activities in EMR systems and Salesforce in compliance with HIPAA.

Qualifications

• High School diploma or GED equivalent required.
• BA/BS degree preferred OR additional experience above the minimum may be considered in lieu of the required education on a year-for-year basis.
• A minimum of three (3) years’ work experience required; working with seniors a plus as is a general understanding of Medicare Advantage.

Responsibilities

• Advocates and connects eligible patients with health plans and community resources to tailor care.
• Proactively reaches out to disenrolled members to understand barriers to care and facilitates their return.
• Collaborates with center leadership, sales teams, brokers and clinical staff to retain patients.
• Ensures seamless transitions and sustained engagement to prevent gaps in care and hospital visits.
• Documents patient information in EMR systems and Salesforce, maintains queues, adheres to HIPAA and regulatory requirements.
