Job summary

ChenMed is seeking a Patient Continuity and Needs Advocate (PCNA) to bridge patients with health plans and services, ensuring eligible patients receive the right care. The role drives retention, partners with center leadership, sales, brokers, and clinical staff, and ensures seamless transitions to minimize hospital visits.

The PCNA will document in EMR systems (Medals and Salesforce) and maintain patient queues, while complying with HIPAA and company protocols.

Qualifications

• High School diploma or GED required.
• BA/BS preferred OR additional experience above the minimum may be considered in lieu of the required education on a year-for-year basis
• A minimum of three (3) years' work experience required; working with seniors a plus as is a general understanding of Medicare Advantage

Responsibilities

• Advocate for patients and connect eligible individuals with health plans and resources.
• Retain and re-engage disenrolled members with personalized outreach.
• Collaborate with leadership, sales, brokers and clinical staff on retention strategies.
• Ensure care continuity and prevent gaps leading to hospital visits.
• Document patient information in EMR and Salesforce; maintain work queues.
• Ensure HIPAA-compliant documentation.
• Perform other duties as assigned.
