Job summary

ArchWell Health is seeking a Care Navigator to warmly welcome members and their families at our St. Louis centers. You will guide them from arrival through checkout, ensuring needs are met and care is coordinated with a multi-disciplinary team.

The ideal candidate shows strong customer service, passion for senior care, and ability to communicate with diverse populations. Bilingual abilities are a plus; on-site work in St. Louis is required.

Qualifications

• Associates degree preferred or equivalent experience.
• Minimum of one year of work experience in a clinical setting.
• Experience working with EMR systems is a plus.

Responsibilities

• Welcome members and guide them through the check-in process.
• Update member information in the EMR system.
• Schedule appointments and coordinate referrals for a multi-disciplinary care team.
• Assist members with filling out paperwork through electronic kiosks.
• Request medical records and upload documentation to EMR.
• Field questions from members and their families.
• Manage phone line and relay messages to the right staff.
• Assist with center events as needed.
