Qualifications

• Associates Degree in Gerontology, Geriatrics, Social Work, Community Services, Business/Public Administration, or related field and three years of gerontological client case management experience in a health or home healthcare environment.

Responsibilities

• Plan and administer daily supportive services for the Frail and Elderly Program.
• Coordinate in-home care and community resources for seniors and caregivers.
• Maintain accurate case files and documentation; prepare required forms and reports.
• Coordinate transportation and act as advocate for clients and families.
• Engage with clients, families, and agencies to ensure service delivery and follow-up.
