Job summary

Bernalillo County is hiring a Community Case Manager to provide case management and counseling across residential, community-based, and jail-based settings. You will assess needs, determine levels of care, and coordinate referrals to treatment and social services.

The role involves working with external agencies, families, and justice system personnel to support client progress and ensure quality service delivery. A valid NM driver’s license and adherence to safety guidelines are required.

Qualifications

• Bachelor’s degree in Criminal Justice, Psychology, Sociology, or a related field.
• One (1) year of work experience in criminal justice, social work, counseling, or a related field.

Responsibilities

• Conducts initial client interviews and gathers case histories to assess needs.
• Coordinates with community agencies to schedule and support client participation in treatment and programs.
• Provides case management services by identifying needs and making referrals for treatment and services.
• Establishes and maintains relationships with community partners to facilitate client support and service delivery.
• Transports clients to appointments and maintains case documentation.
