Job summary

Bernalillo County is seeking a Community Case Manager to provide case management and counseling services for assigned clients in residential, community-based, and jail-based settings.

You will assess needs, determine levels of care, and coordinate referrals to treatment, healthcare, legal, and social services while collaborating with external agencies and families to support client progress.

Qualifications

• Bachelor's degree in Criminal Justice, Psychology, Sociology, or a related field.
• One year of work experience in criminal justice, social work, counseling, or a related field.
• LSAA license preferred.

Responsibilities

• Conduct initial client interviews and gather case histories.
• Coordinate with community agencies to support treatment participation.
• Provide case management by identifying needs and arranging referrals.
• Maintain accurate case documentation and prepare case files.
• Collaborate with families, justice system personnel, and community resources.
• Transport clients to appointments as needed.
