St. Paul’s Senior Services in San Diego is hiring a full-time RN Case Manager to coordinate comprehensive participant care with an interdisciplinary team, perform timely assessments, manage medications, and educate participants per CMS guidelines.

The role serves as a clinical liaison among participants, families, PCCs, providers, pharmacies, and home care services to promote safe, effective, and continuous care, with travel to participant locations as required.

Qualifications

• Current California Registered Nurses License with CPR certification.
• Minimum of one year’s experience with frail or elderly population.
• Valid driver’s license and reliable transportation.

Responsabilités

• Administer medications per nursing standards and document treatments.
• Coordinate participant care with the interdisciplinary team and attend IDT/Plans of Care meetings.
• Conduct comprehensive assessments and develop care plans per CMS guidelines.
• Maintain medical records and complete quarterly nursing notes as needed.
• Determine nursing level of care as delegated by Clinical Services Manager.
• Manage end-of-life case management and in-home nursing visits outside LVN scope.
• Attend in-service training and follow safety policies.
