Volunteers of America Mid-States (VOA) is hiring a Case Manager to assist individuals and families experiencing homelessness who have been approved for RRH or Joint RRH/TH services. You will work with the VOA Home team to connect clients with medical and mental health services, housing, substance abuse services, and social supports.

Qualificações

• Master of Social Work or a related field; or Bachelor of Social Work with >5 years experience; or 9 years experience with no degree.
• Ability to perform family needs assessment, access local resources, advocate, counsel, and coordinate with service providers.
• Excellent verbal and written communication, highly organized, creative problem-solving.
• Must have personal automobile, valid driver's license, insurance, and willingness to travel within the service area.

Responsabilidades

• Outreach to community partners and recruitment of landlords for VOA programs; assess and monitor case plans and follow-up.
• Establish linkages with agencies and service providers to assist participants with health and daily living services.
• Coordinate referrals and services to access health care, housing and supports.
• Educate participants on resources and rights; assist with public benefits and IDs or transit tickets.
• Document case activity promptly and accurately in HMIS and EHR systems.
• Demonstrate clinical judgment and culturally appropriate engagement with families.
