Job summary

Enhabit Home Health & Hospice in Wilmington, MA seeks a qualified Licensed Professional to review and approve patient information across care events. Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Must maintain a valid clinical license per state rules.
• Minimum of one year experience in Medicare home health or hospice.
• CPR certification and a valid state driver license.
• Automobile liability insurance as required by law.
• Preferred experience with packet review / quality improvement and with coding.
• Basic demonstrated technology skills; operation of a mobile device.
• Experience with Homecare Homebase (HCHB) preferred.
• Dependable transportation in good condition; able to drive in all weather.
• Strong oral and written communication; English fluency; active listening.
• Proficient Microsoft Office skills (Word, Excel).

Responsibilities

• Review and approve patient information submitted by the Licensed Professional (LP) at start of care, re-certification, resumption of care, or evaluation visit.
• Partner with branch leadership to educate and train LPs on documentation to meet regulatory standards.
• Provide one-on-one training when needed and promote quality patient care with strong stakeholder relationships.
