Job summary
Nevada Health Centers in Las Vegas, NV, seeks a Quality and Population Health Manager (RN) to lead quality improvement initiatives across patient safety and population health. The role partners with operations, clinical, finance, marketing and outreach to drive performance and ensure compliant care delivery.
The ideal candidate is an experienced RN with at least five years in quality improvement and case management, plus knowledge of EHR systems.
Qualifications
- Graduate of an accredited nursing school.
- Licensure as a registered nurse in the state of Nevada
- Knowledge of FQHC operations, HRSA requirements, NCQA standards, and UDS reporting measures.
- Current CPR or BLS certification
- Minimum of five years of experience in quality improvement and case management activities.
- Working knowledge of Electronic Health Record systems
Responsibilities
- Supports and supervises RN Case Managers
- Coordinate monthly Quality Improvement and QA Committee meetings
- Collaborate on quality improvement initiatives focused on patient outcomes, safety and experience
- Monitor, analyze, and report on quality metrics updates
- Oversee population health programs and case management activities
- Collaborate with health plan quality teams on patient transitions
