Nevada Health Centers is seeking a Quality & Population Health Manager. The registered nurse will lead quality improvement across patient safety and population health initiatives, collaborating with operations, clinical, finance, marketing, and outreach teams.

The role oversees development, implementation, and evaluation of programs to improve clinical outcomes, close gaps in care, enhance patient experience, and support regulatory and value-based care requirements.

Qualifications

• Graduate of an accredited nursing school.
• Nevada RN license or eligibility.
• Knowledge of FQHC operations, HRSA, NCQA, and UDS measures.
• Current CPR/BLS certification.
• Minimum five years in quality improvement and case management.
• Experience with electronic health records.

Responsabilités

• Lead quality improvement across patient safety and population health.
• Support RN Case Managers and coordinate meetings.
• Collaborate on patient outcomes, safety, experience, and performance.
• Monitor quality metrics and report updates.
• Oversee population health programs and chronic disease initiatives.
• Plan and manage department budget and staffing with CNO and leaders.
• Develop policies and staff education for quality initiatives.
• Coordinate data analysis to align with quality metrics.
• Lead national accreditation efforts and quarterly reviews.
• Participate in health plan quality/population health meetings.
