Job summary

Hartford HealthCare at Home seeks a Quality Assurance & Performance Improvement (QAPI) Specialist to advance clinical documentation and outcomes through data-driven improvement initiatives. You will compile trends, lead Performance Improvement Plans, and ensure regulatory compliance, including OASIS and ICD-10 coding.

The role requires 3 years of certified home health experience, knowledge of PDGM and CAHPS, and CT licensure. Travel to branches across the United States may be necessary.

Qualifications

• 3 years of certified home health experience required.
• Value Based Purchasing, PDGM, CAHPS-preferred.
• CT Licensure in field of specialty.
• Licensure, Certification, Registration.

Responsibilities

• Assist and coordinate with comprehensive quality improvement, data analysis and reporting.
• Compile trends and reports quality data in patient care, safety, risk management, infection control, outcomes and customer satisfaction.
• Lead development of Performance Improvement Plans using audit results.
• Review OASIS assessments for compliance and completeness.
• Ensure appropriate ICD-10 coding and sequencing related to patient condition.
• Develop interventions to impact quality scores.
• Promote and participate in QAPI activities and staff education.
• Travel to branches as needed to support regional quality initiatives.
