Job summary

Hartford HealthCare at Home is seeking a Quality Assurance & Performance Improvement (QAPI) Specialist to help improve clinical documentation and outcomes through a data-driven approach. You will compile trends, report quality data across patient care, safety, risk, and infection control, and lead development of Performance Improvement Plans.

You will review records, track data, and support regulatory compliance while promoting best practices and collaboration across care teams.

Qualifications

• 3 years of certified home health experience required.
• Value Based Purchasing, PDGM, CAHPS-preferred.
• Licensure, Certification, Registration.
• CT Licensure in field of specialty.
• Knowledge of federal and state regulatory requirements.
• Strong communication skills and ability to travel between branches.
• Adherence to Hartford HealthCare Leadership Behaviors and Core Values.

Responsibilities

• Assist with data analysis and reporting for quality programs.
• Develop performance measures and improvement plans.
• Provide education and facilitate PI teams using quality improvement methods.
• Communicate effectively in writing and verbally with staff and leadership.
• Travel to branches as needed to support clinical quality initiatives.
