Job summary

Mercy Health is seeking a Registered Nurse for Progressive Care (Step down) at St. Elizabeth Health Center in Youngstown, OH. The RN will lead care coordination between Med-Surg and ICU, assess patient status, and develop care plans to improve outcomes.

Qualifications

• Current state RN license.

Responsibilities

• Collaborate with the interdisciplinary care team.
• Conduct initial and ongoing patient assessment, create a plan of care, implement treatment and evaluate effectiveness.
• Administer medications in a safe manner per policies.
• Coordinate patient care across hospital departments and serve as care coordination contact.
• Support ongoing quality improvement in the department.
