Qualifications

Responsibilities

• Collaborate with the interdisciplinary care team.
• Conduct initial and ongoing patient assessment, analyze data, create a plan of care, implement treatment and evaluate effectiveness.
• Serves as point of contact for patients' care coordination throughout hospital departments.
• Acts as a patient safety advocate by participating in ongoing quality improvement in the department.
• Administer medications in a safe manner consistent with state practice and organization policies.
