Job summary

Mercyhealth is seeking an RN Patient Navigator to coordinate and facilitate patient care, ensuring appropriate services and efficient resource use. The role emphasizes improving care transitions, education, and outcomes, with a Monday–Friday schedule and an on-call rotation.

The candidate will assess patients early, develop individualized plans, and collaborate with nurses and physicians to meet core measures and discharge goals. Strong nursing knowledge and software proficiency are required.

Qualifications

• Graduate of an accredited nursing program.
• Current Registered Nurse license in the state of practice.
• BSN required or agreement to enroll in a BSN completion program.
• Case Management Certification (ACM) preferred; within three years.
• BLS/CPR certification within 90 days

Responsibilities

• Provides case management, advocacy, and care coordination to hospital patients to ensure seamless transitions.
• Conducts early assessment by case-finding and referral to identify discharge needs.
• Creates individualized, patient-centered plan for continuing care needs.
• Makes referrals to post-hospital providers leveraging community resources.
• Collaborates with the healthcare team to ensure timely discharge and patient education.
