Mercy is seeking an RN Navigator to coordinate and personalize care for patients across the healthcare continuum. You will guide patients through complex care pathways, facilitate timely referrals, and collaborate with interdisciplinary teams to reduce barriers and improve outcomes.

You will educate patients and families, support discharge planning, and leverage Electronic Health Records (EHR) to track progress and quality metrics in a collaborative healthcare environment.
