Job summary

Mercyhealth is seeking a Registered Nurse Patient Navigator to coordinate care, facilitate effective transitions, and optimize resource use within our hospital setting. The role emphasizes compassionate, culturally sensitive nursing care and collaboration with the care team.

The RN will conduct initial assessments, develop patient-centered discharge plans, and educate patients and families about chronic conditions and post-acute needs. Weekend/on-call duties may be required.

Qualifications

• Case management and care coordination in a hospital setting.
• Ability to educate patients and families about care plans and chronic conditions.
• Experience with EHR systems and care management workflows.

Responsibilities

• Provides case management, advocacy, and care coordination to ensure seamless transitions and reduce readmissions.
• Conducts early patient assessment to identify coordination and discharge needs.
• Creates individualized plans for ongoing care and coordinates post-hospital referrals.
