Mercyhealth seeks an RN Patient Navigator to coordinate and facilitate patient care, ensuring appropriate services and efficient resource use. The role emphasizes compassionate, culturally sensitive holistic nursing in a clinical setting, with 40 hours/week, Monday–Friday, daytime shift.

Responsibilities include care coordination, discharge planning, patient education on chronic diseases, and collaboration with the care team to meet core measures and Utilization Management standards.

Qualifications

• Graduate of an accredited nursing program.
• BSN or enrollment in a BSN completion program upon hire.

Responsabilités

• Provide case management, advocacy and care coordination to ensure seamless transitions.
• Perform early patient assessment to identify discharge planning needs.
• Create individualized care plans for continuing care.
• Refer to post-hospital providers using community resources.
• Collaborate with healthcare team to ensure timely discharge and complete instructions.
• Educate patients and families about chronic disease conditions.
• Collaborate with staff to meet core measures before discharge.
• Participate in Utilization Management review.
• Identify and mitigate financial risks to patient and organization.
