Qualifications

• RN licensed in state of practice.
• BSN degree required or completion plan in progress.
• Case management certification preferred within three years.

Responsibilities

• Provide case management, advocacy, and care coordination to hospital patients.
• Conduct early patient assessment to identify coordination and discharge planning needs.
• Create individualized plans for continuing care needs and coordinate post-hospital referrals.
• Collaborate with healthcare team to ensure timely discharge and comprehensive patient education.
• Educate patients and families about chronic conditions with current nursing knowledge.
• Ensure core measures are met prior to discharge and participate in Utilization Management.
