Mercy is seeking a Nurse Navigator to lead patient care coordination, provide pre-op education, and guide care teams in an acute and post-acute setting. The role emphasizes safety, quality, and adherence to Mercy Service Standards while supporting discharge planning and utilization management.

The ideal candidate will be a registered nurse with 2–3 years of experience, current state licensure, and BLS/ACLS certifications within 90 days of hire. BSN is preferred.

Qualifications

• RN license in state of practice
• Graduate from approved nursing program
• BSN preferred for the role
• 2–3 years of nursing experience required
• BLS and ACLS certifications within 90 days of hire/transfer

Responsibilities

• Provide pre-op education and daily inpatient rounds.
• Coordinate discharge planning and post-acute care.
• Collaborate with bedside nurses to align care plans with regulatory guidelines.
• Support quality, safety, and financial components of care and performance metrics.
