Job summary

Mercy is seeking a Nurse Navigator to lead and coordinate patient care across inpatient and outpatient settings. You will assess care needs, guide the plan of care with bedside nurses, and promote timely, safe, and high-quality outcomes backed by Mercy standards.

Qualifications

• Graduate from approved school of nursing.
• RN licensure in state of practice.
• 2-3 years of nursing experience required.
• BSN preferred.

Responsibilities

• Coordinate the patient’s plan of care with the bedside nurse.
• Assess care requirements and make recommendations based on illness severity and service needs.
• Deliver pre-op education and conduct daily rounds while hospitalized.
• Support discharge planning for post-acute services and LOS management.
• Promote high-quality, safe care in alignment with Mercy standards.
• Mentor and guide direct care staff within the unit.
